Abstract

PEIT is now widely used in treatment of hepatoma. We applied this technique for thyroid cancer and assesed its efficacy. We have treated twenty patients with papillary carcinoma and ,one with medullary cacinoma. Fifteen of them having recurrent tumor after total thyroidectomy showed no response to 1-131 therapy, and external irradiation also failed in one. Four had unresectable primary tumor. We obtained informed consent and then measured volume of target mass by US or CT. Total injection dose of ethanol was planned to be 100-120% of initial mass volume. After local anestheitization, ethanol was injected using fine needle with the guide of US or CT. Treatment was repeated at moderate intervals until injected volume reached to that of initial planning. Histopathological examination proved that ethanol-permeated part of the tumor became direct necrosis with dense fibrosis. Therapeutic results in 25 lesions from our materials remission. In others, at least more than 30 % of volume regression occured. From our preliminary experience, it seemes that PEIT has many advantages ; techincal simplicity, certainty of tumor regression, toxity or bone marrow suppression, and little fear of anaplastic transformation. Although some peculia complications like drucnkeness, pain at injection and jrujury of surround normal tissue by leaked ethanol are associated, they are not severe and can be relieved by poper medication. In conclusion, PEIT is highly effective as a pallative therapy for refractory thyroid cancer.
